Job summary

Cowlitz Family Health Center seeks a Behavioral Health Consultant to join a primary care team. You will deliver brief, evidence-based behavioral health services, coordinate care, and support patients with psychoeducation and engagement strategies.

The role emphasizes collaboration with providers, attention to safety, and continuity of care in a fast-paced community setting. Washington licensure is required and some duties may occur via telehealth.

Qualifications

• Master’s or Doctorate degree in social work, marriage and family therapy, psychology, behavioral health or related field.
• Demonstrated ability to collaborate effectively in a team setting.
• Ability to maintain effective and professional relationships with clients and care team.
• Strong communication skills.
• Ability to manage a high volume of patients in primary care.
• Ability to assess quickly and make decisions.
• Competent in assessing suicide lethality and following CFHC protocol.
• Ability to work under pressure and engage clients.

Responsibilities

• Provide integrated behavioral health services within primary care.
• Conduct brief evidence-based interventions and psychoeducation.
• Coordinate care with providers and community resources.
• Utilize warm handoffs and video or phone visits to engage patients.
• Support treatment planning, referrals, relapse prevention and adherence.
