Job summary

PACE Southeast Michigan is seeking a Behavioral Health Specialist to join our interdisciplinary team. You will perform biopsychosocial assessments, provide psychotherapy and crisis intervention, and collaborate with Primary Care and Psychiatry to coordinate patient-centered care for participants in the PACE program.

The role requires a Master’s in Social Work and state licensure in Michigan, with experience in integrated behavioral health and IDT collaboration.

Qualifications

• Master’s Social Worker credential required and licensed to practice in Michigan.
• Knowledge of DSM-5 and clinical treatment approaches within a PCBH integrated care model.
• Experience with mental illness, co-occurring disorders, and primary health care integration.
• Ability to learn and use electronic health record systems and communicate effectively.
• Familiarity with state and federal regulations affecting behavioral health services.

Responsibilities

• Conduct comprehensive biopsychosocial assessments and crisis interventions.
• Provide individual and group psychotherapy and substance use treatment as part of an IDT.
• Coordinate care with Primary Care and Psychiatry as a liaison.
• Document care in electronic health records and ensure accuracy.
• Contribute to staff development and training on behavioral health topics.
• Participate in committees and policy improvement efforts.
