Responsibilities

• Participate in weekly supervision.
• Complete comprehensive individualized assessments that identify psychosocial factors relevant to management of physical health problems including mental health and substance use.
• Utilize screening tools such as ASAM, DAST, AUDIT, PHQ-9, GAD-7 to determine appropriate levels of care.
• Coordinate with multidisciplinary team to develop patient-centered treatment plans with clinical summary, measurable goals, and objectives.
• Monitor patient progress toward identified goals and objectives.
• Provide evidence-based interventions as needed: motivational interviewing, problem solving, coping skills training, mindfulness techniques, cognitive restructuring, and communication skills training.
• Deliver brief interventions and short-term counseling for depression, anxiety, trauma, grief/loss, addiction and related concerns.
• Work with patients in crisis, providing rapid intervention, risk assessment, de-escalation, safety planning, and care coordination.
• Support connections to existing mental health and substance use treatment services.
• Connect patients to community agencies addressing homelessness.
• Serve as team content expert on behavioral changes, substance use issues and community resources.
• Complete all required documentation within organization timelines.
• Maintain strict confidentiality per laws and policies.
• Participate in continuing education, trainings, and community events.
• Receive clinical guidance as needed.
• Adapt to fluctuating patient flow and acuity while maintaining quality.
