Qualifications

• Master’s degree in Social Work, Counseling, Psychology, or related field from an accredited university.
• Three (3) years’ post-graduate experience with behavioral health patients in clinical settings.
• Current Arizona Behavioral Health License (LPC, LCSW, or Clinical Psychologist) in good standing.
• Current Basic Life Support (BLS) certification.
• Level I fingerprint clearance card (or applied within seven working days).

Responsibilities

• Provides focused clinical services including assessment, intervention, and coordination of care for behavioral health concerns.
• Provides interruptible and accessible services to support referral and warm hand-off requests.
• Cultivates education and self-management skills in patients with comorbid conditions.
• Collaborates with primary care providers and interdisciplinary teams to address health conditions.
• Reaches out to Specialty Behavioral Health to staff cases and promote continuity of care.
• Maintains accurate electronic health records in compliance with HIPAA and policies.
• Provides education to care teams on behavioral health assessment and intervention.
• Attends meetings and participates in ongoing training activities.
• Offers crisis navigation and engages mobile crisis teams when needed.
• Participates in the Behavioral Emergency Response Team (BERT).
