Job summary

Southwest Behavioral & Health Services in the Phoenix area is seeking an Integrated High Needs Coordinator I to join the Mesa Outpatient Clinic. This full-time role provides integrated case management to a caseload of clients, advocating for access to services and coordinating with community providers to improve wellness outcomes.

The position requires a Bachelor's degree in psychology or related field and 1–2 years of behavioral health experience, with fingerprint clearance and a valid AZ

Qualifications

• Bachelor's degree with 1-2 years of behavioral health experience.
• Knowledge of DSM, crisis intervention and community resources.
• Experience coordinating with multi-disciplinary teams.

Responsibilities

• Facilitates CFT meetings for assigned clients.
• Develops ISP’s using information from assessments and team input.
• Provides information and support for treatment planning and community linkages.
• Develops safety/support plans for families as needed.
• Maintains accurate progress notes and confidentiality per HIPAA.
