Job summary

Southwest Behavioral & Health Services is a large nonprofit provider of behavioral health services with over 115 sites in the Phoenix area and beyond. The Care Coordinator will engage members, learn their stories, and help improve their quality of life through individual and group interventions, care planning, and coordination with the clinical team.

Qualifications

• Bachelor’s degree in Psychology or related field and 1 year of behavioral health experience.
• Fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive upon hire.
• 3 weeks of PTO in the first year, with increased accruals after continued service; 10 paid holidays.

Responsibilities

• Coordinate consumer care and develop service plans with consumers.
• Assist with referrals, admission, discharge and out-of-home placements; collaborate with clinical team.
• Provide face-to-face crisis intervention and safety planning.
• Maintain accurate and current documentation of contacts and services.
• Travel to east or west valley 1x/week; most duties in office.
