Job summary

Southwest Behavioral and Health Services is seeking an Integrated High Needs Care Coordinator to join our Payson team. You will work with children and families as part of an interdisciplinary group to arrange integrated case management for clients with complex needs.

The role involves facilitating family meetings, coordinating care, linking clients to community resources, and documenting progress while ensuring confidentiality and HIPAA compliance.

Qualifications

• Minimum requirements include a high school diploma or GED and five years of integrated/behavioral health experience.
• Bachelor’s degree in a related field and minimum 1 year of integrated/behavioral health experience preferred.

Responsibilities

• Facilitates family meetings and develops ISP plans using assessments and team input.
• Provides information and support to youth and families on treatment planning and community supports.
• Ensures team meetings in the family’s natural setting to meet needs.
• Develops and revises safety/support plans for families.
• Maintains accurate documentation of contacts and treatment progress.
• Maintains effective communication with clients, healthcare providers, families and staff.
• Notifies supervisor of challenges such as non-compliance or crisis.
