Job summary

Southwest Behavioral & Health Services in Arizona is hiring an Integrated High Needs Coordinator I to join our Mesa Outpatient Clinic. You will work as part of an interdisciplinary team, facilitate CFT meetings, develop ISP plans, coordinate care, maintain documentation, and connect youth and families with community resources to improve wellness outcomes.

A Bachelor’s degree and 1–2 years of behavioral health experience are required.

Qualifications

• Bachelor's degree in Psychology or related field.
• 1 year of full-time behavioral health experience.
• Fingerprint clearance or eligibility; valid driver's license (AZ).
• Knowledge of DSM, community resources, crisis intervention; HIPAA awareness.

Responsibilities

• Facilitates CFT meetings for assigned clients.
• Develops, implements, and revises ISP plans using comprehensive assessments and team input.
• Provides information, education and support on treatment planning and coordinating care.
• Maintains accurate documentation of contact, progress and services.
• Coordinates with PCPs, mental health providers, families and community resources.
• Provides information to CFT members on community resources and linkages.
• Supports youth and families in securing needed services.
• After-hours on-call rotation as needed.
