Job summary

Hebrew SeniorLife Hospice Care, Inc. is seeking an Intake Coordinator to coordinate referrals, screen clinical information, and ensure appropriate orders and discharge paperwork are received.

You will work with the Billing Department to obtain initial authorizations, enroll patients in telehealth, and support scheduling and call triage, with on-site responsibilities and Medicare regulatory familiarity.

Qualifications

• RN/LPN certification with home care experience and Medicare knowledge.
• Experience identifying and coding primary/secondary home care diagnoses.
• Experience obtaining insurance authorizations and telehealth knowledge.
• Proficient with EMR and data entry; strong communication skills.

Responsibilities

• Answer and direct calls on the main home care line and triage inquiries.
• Coordinate referrals, collect clinical info, and enter data into EMR.
• Verify patient insurance and coordinate initial authorizations with Billing.
• Track ongoing authorizations and share clinical updates with staff.
• Enroll patients in Telehealth and monitor readings with clinical staff.
• Notify liaisons and managers of referral status and any issues.
• Assist in scheduling patient visits with Scheduler and Clinical Managers.
