Job summary

Beth Israel Lahey Health at Home is seeking a Hospice and Palliative Care Intake Associate in Beverly, MA for a part-time day shift. You will coordinate referrals, collect essential patient information, and initiate hospice and palliative consults in collaboration with the multidisciplinary team.

Responsibilities include communicating with referral sources, scheduling NP visits, managing EMR workflows in Epic, and ensuring timely processing of approvals and documentation while maintaining HIPAA

Qualifications

• Excellent observation and problem solving/critical thinking skills.
• Strong verbal and written communication skills; proficient computer skills to work with EMR.
• Knowledge of Excel and familiarity with Epic EMR systems is preferred.

Responsibilities

• Respond to referrals and gather accurate patient information to initiate hospice/palliative consults.
• Coordinate referrals with multidisciplinary teams and communicate priorities to care teams.
• Schedule and confirm Face-to-Face visits with the Nurse Practitioner.
• Process admissions and related paperwork in the EMR; maintain HIPAA compliance.
