Hebrew Senior Life in Dedham, MA is seeking an Intake Coordinator to manage referrals, verify insurance, and support the Telehealth program. You will screen clinical information, collect documentation, and coordinate initial authorizations while collaborating with Billing and Clinical staff.

Strong communication and EMR data entry skills are essential. The role includes answering calls, triaging patient inquiries, and assisting Scheduling in staff deployments.

Competenze

• RN or LPN license required.
• Certified Home Care experience - 1–2 years including Medicare regulations.
• Experience identifying and coding home care diagnoses.
• Experience obtaining insurance authorizations.
• Telehealth experience.
• Proficient computer skills with EMR data entry.
• Strong verbal and written communication skills.
• Motivated to learn and flexible to change.
• Prior Intake/Office Nurse experience preferred.

Mansioni

• Answer and direct calls on the main home care line.
• Triages patient inquiries and assists scheduling as needed.
• Obtain referral information from sources via phone and Allscripts.
• Maintain ongoing communication with Home Care Liaison Team.
• Enter referral data into EMR, including diagnoses coding.
• Obtain initial documentation and discharge paperwork from sources.
• Verify primary insurance and coordinate initial authorizations.
• Track ongoing authorizations and share required clinical info.
• Notify managers of referrals and clinical issues.
• Maintain patient medical records per policy.
• Enroll patients in Telehealth and monitor readings.
• Cover Intake responsibilities on holidays as required.
