Job summary

Trinity Hospice is seeking a Hospice Intake Coordinator to facilitate referral intake, coordinate care with the interdisciplinary team and patients and families. You will create and maintain patient charts in Consolo EMR, verify insurance benefits, and ensure all orders and forms are uploaded.

This role supports ongoing marketing and staff collaboration, with strong emphasis on communication and documentation accuracy.

Qualifications

• One year general office experience in the healthcare industry.
• Experience in the hospice space preferred.
• Proficient with Google Suite.
• Knowledge of medical terminology.
• Strong interpersonal and organizational skills.
• Valid and current auto liability insurance.
• High school diploma required; Bachelor's preferred.

Responsibilities

• Receives and documents new referrals in a timely manner.
• Verifies Medicare, Medicaid, and private insurance benefits timely.
• Ensures physician orders, H&Ps and transfer docs are provided for clinical approval.
• Creates patient chart in EMR (Consolo).
• Notifies clinical team of pending admissions.
• Uploads all orders, docs, and intake forms to the patient chart.
• Maintains referral log and End of Day report daily.
• Verifies benefits for private insurance patients monthly.
• Assists in ordering equipment as directed by the DON/PCMs.
• Attends weekly/monthly marketing and staff meetings.
• Prepares consent and admission packets.
• Provides effective communication to patients, staff, and referral sources.
• Participates in the Agency’s QAPI program.
