Qualifications

• High school diploma or equivalent is required; a bachelor’s degree is preferred.
• 3–5 years of related experience is desirable.
• Strong verbal and written communication skills with empathy and professionalism.
• Excellent attention to detail and organizational abilities.
• Ability to assess needs and refer to appropriate resources or programs.
• Proficient with Outlook, Teams, databases, and internal systems.
• Ability to manage multiple tasks and maintain accurate documentation.

Responsibilities

• Conduct initial assessments to understand neighbor needs and determine eligibility.
• Collect and maintain accurate, confidential records of interactions.
• Connect neighbors to internal programs and external resources.
• Provide clear information about JBNC services and partners.
• Enter and manage data in internal systems with timely documentation.
• Follow up to ensure successful connections to services.
• Coordinate referrals with internal teams and external partners.
• Participate in outreach and community events to promote services.
• Contribute to program development and evaluation for continuous improvement.
