Responsibilities

• Accepts and processes referrals per agency policy in collaboration with Intake Nurses.
• Coordinate intake information for referrals and service management.
• Assist with non-clinical tasks: data entry, filing, phones, referrals, and supplies.
• Enter referral information into the agency health record with payers listed.
• Participate in orientation of new staff members.
• Collect pertinent information for face-to-face documentation requirements.
• Collaborate with Intake Nurse for final acceptance of complex referrals.
• Maintain databases and information for the Intake Department.
• Work on special projects as needed.
• Communicate with referral sources and branch personnel with a customer-service focus.
• Work weekends/holidays as rotating duties.

Intake Specialist

Day Shift / 40hrs/wk

Shift: 9:00am - 5:30pm

*This role will be a combination of on-site in Wallingford, CT and remote*

Accepts and processes referrals, completes full data entry of referral, obtains and verifies insurance information and requests initial authorization for Home Health and Hospice admissions.

Essential Duties and Responsibilities:

• Accepts and processes appropriate referrals to the agency per agency policy in collaboration with Intake Nurses and coordinates all intake information in the management of referrals for service.
• Assures coordination of all required services to clients and families by agency staff and community resources.
• Assists with non-clinical support including, data entry, filing, answering phones, calling referral sources, and ordering supplies.
• Enters referral information into the agency health record including listed payers.
• Participates in the orientation of new staff members.
• Initiates collection of pertinent information related to face-to-face documentation requirements.
• Collaborates with Intake Nurse for final acceptance of complex referrals.
• Maintains databases and pertinent information relative to the needs of the Intake Department.
• Works on special projects as needed.
• Communicates effectively and tactfully with multiple referral sources, maintaining a customer service focus with outside agencies as well as branch personnel and colleagues.
• Will report to work on time, use time off appropriately, and complete annual education & medical requirements. Works weekends and holidays on a rotating basis as determined by department needs.
• Participates in Weekend and Holiday rotation as needed and required.
• Perform other duties as required.

MINIMUM QUALIFICATIONS:

• High school Diploma or equivalency
• Experience with computer data entry, word processing, and creation of reports.
• Maintains current drivers’ license and auto insurance.
• Possess reliable transportation (Exception: Employees who utilize public transportation—Supervisory approval required).
• Previous experience with medical terminology or employment in a medical setting preferred.
