Job Summary
The Intake Coordinator is responsible for managing new patient referrals, contacting newly accepted patients, verifying patient information, and maintaining accurate intake records.
The Intake Assistant reports to the Department Lead, Administrator, and/or Director of Nursing (DON).
Key Responsibilities
Patient & Customer Support
- Answer phone calls and messages promptly.
- Contact newly accepted patients and introduce the organization.
- Verify patient information provided in referrals.
- Answer patient and customer questions professionally.
- Provide clear and accurate information.
- Handle complaints and escalations with professionalism and empathy.
- Work to resolve issues promptly.
Referral & Intake Processing
- Monitor and maintain incoming faxes.
- Print and organize accepted referrals.
- Verify referral information for accuracy and completeness.
- Enter patient and referral information into the appropriate system.
- Keep the referral log accurate and up to date.
- Ensure all accepted referrals are entered into the referral log.
- Distribute patient charts and information promptly for further processing.
Communication & Teamwork
- Maintain professional and positive working relationships with all departments.
- Communicate effectively with team members and leadership.
- Contribute to a professional, positive, and motivated work environment.
- Use assigned tools, resources, and materials on a daily basis.
- Perform additional duties as assigned by management.

Qualifications
- Working knowledge of home health requirements and processes.
- Experience working in a fast-paced healthcare or administrative environment.
- Experience using an Electronic Medical Record (EMR) system.
- Previous experience with patient intake, referrals, or healthcare administration is preferred.
Position Details
Job Type: Full-time
Pay: $21.00-$25.00 per hour
Medical Specialty: Home Health
