Job summary

Masonicare Home Health & Hospice in Wallingford, CT is seeking a Referral Intake coordinator for a Day Shift, 40 hours per week. You will accept referrals, verify information, and request initial authorizations for admissions, with a mix of on-site and remote work.

Responsibilities include coordinating services, data entry, and supporting intake staff, while ensuring timely communication with sources and colleagues. Requires a high school diploma and basic office tech skills.

Qualifications

• High school diploma or equivalent required.
• Experience with data entry, word processing and reporting.
• Must maintain a valid driver’s license and auto insurance.
• Reliable transportation required for assignments.

Responsibilities

• Accept and process referrals per agency policy in collaboration with Intake Nurses.
• Coordinate services for clients with staff and community resources.
• Perform non-clinical tasks: data entry, filing, phones, sourcing referrals, supplies.
• Enter referral data into the health record with payer details.
• Assist with staff orientation and leave documentation requirements.
• Collaborate with Intake Nurse for complex referrals and maintain intake databases.
• Support weekend/holiday coverage as needed.
