Qualifications

• Master's degree in Social Work, Counseling, Psychology, or a related mental health field.

• Proficient in documenting and data management.

Responsibilities

• Conduct behavioral health screenings for patients in the ED, intake, and hospital settings.
• Gather demographic, clinical, and insurance information to support care coordination.
• Assist with insurance verification and pre-certification processes.
• Coordinate assessments, admissions, and transfers with physicians and care teams.
• Provide crisis support and connect patients with community resources.
• Document patient information accurately in EMR and Access Center databases.
• Maintain confidentiality and follow safety and quality guidelines.
