Qualifications

• 2–5 years in intake/referrals in home health preferred.
• In-person office setting Monday–Friday with weekend availability as needed.
• Strong written and oral communication; excellent customer service.
• Team leadership; organized and flexible to changing priorities.

Responsibilities

• Plans, organizes and directs staff assignments to complete intake tasks timely and accurately.
• Manages performance development and disciplinary actions for intake + scheduling staff.
• Maintains positive relationships with agency stakeholders and members.
• Provides training on changes, process improvements or feedback.
• Oversees new hire training for intake staff.
• Responds to calls to handle referrals, scheduling, and coordination of details.
• Coordinates field staff by gathering information and supporting workflows.
• Manages Homecare Homebase workflow for Intake, Scheduling or Back-office teams.
• Pulls and reviews intake reporting to meet admissions goals and scheduling workflow.
• Collaborates with agency leadership for additional office support.
