Job summary

The Pennant Group, Inc. is seeking an Intake Coordinator to manage all aspects of patient intake and referral processes. You will communicate with patients and families, perform data entry, and oversee insurance verification and authorizations to ensure smooth admissions for home health and hospice services.

You will build relationships with referral sources, coordinate education for patients, and drive adherence to regulatory requirements and company policies across the intake team.

Qualifications

• Healthcare experience, preferably in referrals/intake for home health or hospice.
• Strong communication, negotiation, and public relations skills.
• Autonomy, assertiveness, flexibility and cooperation in performing job responsibilities.
• Understanding of regulatory requirements and accreditation standards.

Responsibilities

• Directs daily patient referral and intake operations and oversees intake policies.
• Ensures compliance with state, federal, and Joint Commission requirements.
• Oversees patient education, plan of care initiation, and coordination with multiple providers.
• Manages insurance verification and authorization processes to maximize reimbursement.
• Maintains relationships with referral sources and coordinates transitions to home health/hospice.
• Assists in budget preparation and resource allocation for the intake department.
• Monitors referral statistics and strengthens community perceptions of the provider.
