Job summary

Qualifications

• At least two years of experience in a social service-based program is preferred.
• Strong communication and organizational skills required.

Responsibilities

• Obtain household information, record it in software, and prioritize needs.
• Identify services and coordinate referrals with providers.
• Enter data, maintain files, schedule appointments, and conduct interviews.
• Follow guidelines and maintain confidentiality and records as required.
• Assist teammates and respond to inquiries in a timely manner.
