Job summary

By The Bay Health, a nonprofit hospice provider, is seeking an Intake-Information Coordinator to ensure compliant intake processes and accurate data collection for referrals.

The full-time role in a hybrid setting combines on-site work at the Larkspur office with one remote day, following a Monday–Friday schedule and 8:30am–5:00pm hours.

Qualifications

• Associate degree (A.A.) or equivalent from a two-year college or technical school; or equivalent combination of education and experience.
• Minimum two years’ experience performing clerical duties preferably in a healthcare setting.

Responsibilities

• Coordinate new intake referrals, gather medical records, hospice orders, and patient insurance information.
• Perform data entry, intake into EMR for all referrals received.
• Collect data from referral sources, family, and facilities as needed to complete pre-admission information.
• Follow up on information required for referral and admission to By the Bay Health.
• Schedule hospice evaluation/informational and admissions visits efficiently.

Schedule:

Monday - Friday;8:30am - 5:00pm

Location:

Hybrid - 4 days onsite at our Larkspur office, 1 day remote
