Job summary

By The Bay Health, a nonprofit hospice provider, seeks a Registered Nurse to serve as Hospice Nurse Case Manager. The role delivers skilled nursing assessment, planning, and home-based care to maximize comfort for patients and families.

Schedule is Monday–Friday, 8:30am–5:00pm. CA RN license required; on‑site work with onboarding, mentorship, and interdisciplinary team support across counties. COVID vaccine compliance noted.

Qualifications

• California RN license is required; Public Health Nursing certification desirable.
• HPNA certification preferred.
• AA or BS in Nursing from an accredited program.
• Experience in acute care/home health/hospice is preferred but not required.

Responsibilities

• Provide home-based nursing care as part of an interdisciplinary team.
• Coordinate patient/family services and implement the plan of care.
• Support the team with education and resources as needed.

Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families.

We are currently seeking a Registered Nurse to serve as a Hospice Nurse Case Manager. This person will be responsible for the provision of skilled nursing assessment, planning and care to maximize the comfort and health of patients and families.

Schedule

Monday – Friday; 8:30am – 5:00pm

Location

Essential Duties & Responsibilities include (but are not limited to):

• Provide care in the home and work as a member of the interdisciplinary team comprised of nurses, social workers, chaplains, hospice aides and physicians.
• Coordinate all patient/family services and prioritization of needs, including implementation of the individualized plan of care.

Education/Experience

• AA or B.S. degree in Nursing from an accredited Nursing School
• Experience in acute care/ home health and/or hospice setting preferred but not required.

Certificates, Licenses, Registrations

• Current license as a registered professional nurse in the State of California.
• Public Health Nursing certification highly desirable.
• HPNA certification preferred.
• Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.

Pay Range

$65.70 – $75.70 hourly
