By The Bay Health in Sonoma, CA is seeking a Healthcare Provider Relations professional to grow our network, educate hospitals and providers, and promote the benefits of community-based hospice. This role emphasizes relationship development, admissions facilitation, and teamwork within a respected non-profit organization.
Primary duties include outreach to referral sources across Sonoma and Napa Counties, contract coordination, and reporting on outreach activities.
