Qualifications

• High school diploma or GED required.
• Associate degree or higher in a related field preferred.
• Prior experience in case management, social services, intake coordination, or a similar role preferred.
• Familiarity with intake procedures and participant engagement strategies preferred.
• Lived experience relevant to the population served is valued.
• Active, restriction-free Washington State driver’s license required, with an acceptable driving record.

Responsibilities

• Serve as the first point of contact, providing a welcoming experience and an overview of available programs.
• Conduct intake assessments, gather required information, and complete eligibility screenings.
• Ensure all completed consent forms and documentation meet organizational and regulatory requirements.
• Provide service information and connect individuals to appropriate programs based on identified needs.
• Coordinate and schedule appointments across internal services, including case management, counseling, and medical care.
• Facilitate internal and external referrals and assist participants in accessing services.
• Act as a liaison between participants and providers to ensure prompt access and warm hand-offs.
• Maintain ongoing contact to encourage engagement, especially during early care milestones.
• Conduct outreach to individuals who miss appointments, assist with rescheduling, and document follow-up efforts.
• Identify barriers to engagement and communicate findings to the care team.
• Track activity metrics and participate in team meetings to support coordinated care.
• Ensure documentation complies with HIPAA and 42 CFR Part 2.
• Support community outreach and maintain knowledge of community resources.
