Job summary

Qualifications

• High school diploma or GED required.
• Associate degree or higher in a related field preferred.
• Prior experience in case management, social services, intake coordination, or a similar role preferred.
• Lived experience relevant to the population served is valued.
• Washington State driver’s license required with acceptable driving record.

Responsibilities

• Serve as the first point of contact with a welcoming experience and program overview.
• Conduct intake assessments, gather information, and complete eligibility screenings.
• Ensure consent forms and documentation meet requirements.
• Coordinate scheduling across services and facilitate referrals.
• Maintain participant engagement during early care and milestones.
• Document interactions in the EHR and track activity metrics.
• Collaborate with teams to support coordinated care and outreach.
