Job summary

Evergreen Treatment Services is seeking a Screening and Outreach Coordinator in Burien, WA. The role supports LEAD referrals, conducts street outreach, and coordinates with law enforcement and service providers to engage participants.

Strong outreach experience and commitment to housing, health, and harm-reduction services are essential. The position reports to the REACH team and requires collaboration across partners to ensure timely intake, risk assessment, and outcomes.

Qualifications

• High school diploma or equivalent required; undergraduate degree preferred; relevant experience may substitute for degree.
• Demonstrated street-based outreach and engagement with individuals who have experienced trauma and marginalization.
• Experience building collaborative relationships with law enforcement and social services to serve mutual clients.
• Understanding of substance use disorders and harm reduction; commitment to serving people experiencing homelessness.

Responsibilities

• Serve as the primary contact for referrals to LEAD.
• Screen and outreach to referred individuals; respond to urgent needs (medical care, shelter, food, clothing).
• Develop relationships with LEAD partners and other service providers to coordinate services.
• Enter and track referrals in the agency database; update demographics and assignments.
• Coordinate with police and sheriff offices; respond to officers when case managers are unavailable.
• Attend meetings and document activities per contract requirements.
