Job summary

Qualifications

• High School Diploma or equivalent or equivalent experience.
• Experience in case management, assessment counseling, and crisis intervention.
• Strong verbal and written communication skills.
• Ability to work in a fast-paced, demanding environment.
• Resourceful and detail-oriented with strong organizational skills.
• Computer literacy, particularly with Microsoft Office applications.

Responsibilities

• Assess all new clients for program eligibility.
• Provide clear explanations of Homebase services and collect initial documentation.
• Verify housing crisis via housing court stipulations, rent demands, subsidies, etc.
• Assist clients with completing intake forms and service plans; enter data.
• Refer clients to external agencies for financial assistance, legal services, and public benefits.
• Refer clients to housing/housing court workshops and related services.
• Participate in program outreach by presenting the Homebase program.
• Establish and maintain relationships with internal and external agencies.
• Prepare regular reports and ensure client records comply with policies.
