Job summary

Choice Health At Home in Tyler, TX is seeking an Intake Coordinator to manage patient referrals and act as the liaison between hospitals, physicians, families and clinical staff. This full-time role ensures a seamless start of care by verifying benefits and entering data into EMR.

You will document referrals promptly, verify payor sources, coordinate with Nursing Supervisors and the interdisciplinary team, and communicate effectively with patients and referral sources.

Qualifications

• Minimum one year of experience in referral intake.
• Experience with third party payers, Medicare and Medicaid desirable.
• Medical field background with familiarity with medical terminology.
• Excellent computer and customer service skills.
• LVN/LPN preferred.
• Valid Driver’s License.
• Valid Auto Insurance.
• HomeCare HomeBase experience preferred.

Responsibilities

• Receives and documents new referrals promptly.
• Verifies payor sources and tracks insurance authorizations.
• Coordinates care with Nursing Supervisors and the interdisciplinary team.
• Provides clear communication to patients and referral sources.
• Responds to agency and patient needs professionally.
• Participates in agency training and QAPI programs.
• Supports quality care and aligns with agency policies.
• Acts as liaison between hospitals, physicians, families, and staff.
