Job summary

Choice Health At Home is hiring an Intake Coordinator in Venice, FL to manage initial referrals and serve as the liaison between hospitals, physicians and clinical staff. You will verify insurance, obtain authorizations, and enter patient data into EMR systems.

The role requires experience in referral intake, knowledge of third-party payers, and strong communication skills to coordinate care with the nursing team and patients. A valid license and HomeCare HomeBase familiarity are preferred.

Qualifications

• Minimum one year of referral intake experience.
• Experience with third party payers, Medicare/Medicaid desirable.
• Familiarity with medical terminology.
• Excellent computer and customer service skills.
• Valid Driver’s License and Auto Insurance.
• HomeCare HomeBase experience preferred.

Responsibilities

• Receives and documents new referrals promptly.
• Verifies payor sources and tracks insurance authorizations.
• Coordinates care with Nursing Supervisors and the interdisciplinary team.
• Communicates with patients, staff and referral sources.
• Supports participation in the Agency's QAPI program.
• Promotes agency policies and quality of care.
