Job summary

Enhabit Home Health & Hospice is seeking a coordinated intake/ referrals role in Tyler, TX. This position coordinates patient referrals, works with branch and sales teams to ensure smooth transition to services, and applies Medicare guidelines to daily operations.

The role requires a high school diploma or equivalent, 2+ years in health care with 1 year in home health or pediatrics, strong customer service, and basic tech skills including mobile devices. A nursing degree/licensure is preferred.

Qualifications

• Must have high school diploma or equivalent.
• Two+ years in health care with one year in home health or pediatrics.
• Experience in customer service.
• Knowledge of Medicare guidelines for home health/hospice.
• Basic tech skills, including using a mobile device.

Responsibilities
