Job summary

Choice Health At Home is seeking an Intake Coordinator for its Ocala, FL Home Health branch. This full-time role manages the initial patient referral process, acting as the key liaison between hospitals, physicians, families, and clinical staff to ensure a seamless start of care.

You will receive referrals, verify insurance benefits, obtain authorizations, and accurately enter patient data into the EMR system.

Qualifications

• Minimum one year of experience in referral intake preferred.
• Experience with third party payers, Medicare and Medicaid desirable.
• Medical field background preferred with familiarity with medical terminology.
• Excellent computer and customer service skills.
• LVN/LPN preferred.
• Valid Driver’s License and Auto Insurance required.
• HomeCare HomeBase experience preferred.

Responsibilities

• Receives and documents new referrals promptly.
• Verifies patient payor sources and tracks insurance authorizations and initial certifications.
• Coordinates care with Nursing Supervisors, patients, and the interdisciplinary group.
• Communicates effectively with patients, staff, health care professionals and referral sources.
• Responds professionally to agency and patient needs and participates in training and QAPI.
