Job summary

Enhabit Home Health & Hospice in Springfield, IL is seeking a patient referrals coordinator to join our team. You will coordinate patient referrals, support branch and sales teams, and ensure smooth transitions to services while applying regulatory guidelines.

Ideal candidates will have a high school diploma, 2+ years in health care and 1 year in home health or pediatrics, strong customer service, Medicare knowledge, and basic tech skills with a mobile device.

Qualifications

• Must have high school diploma or equivalent.
• At least two years of experience in the health care industry and one year in home health or pediatrics.
• Experience in customer service.
• Knowledge of Medicare guidelines that govern home health or hospice.
• Basic demonstrated technology skills, including operation of a mobile device.

Responsibilities
