Job summary

Enhabit Home Health & Hospice is seeking a patient referrals coordinator who will work to ensure smooth transitions to services. The role emphasizes coordination with branch and sales teams, adherence to regulatory guidelines, and strong customer service skills.

The ideal candidate will have a HS diploma, experience in health care (home health or pediatrics preferred), and familiarity with Medicare guidelines. Valid driver's license and reliable transportation are expected.

Qualifications

• Must have a high school diploma or equivalent.
• Must have at least two years of experience in the health care industry and one year experience in home health or pediatrics.
• Must have demonstrated experience in customer service.
• Must have demonstrated knowledge of Medicare guidelines that govern a home health or hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with the branch and sales teams to ensure adequate coordination and transition to services.
• Apply and demonstrate knowledge of regulatory guidelines and governing processes.
