The role requires strong customer service, a high school diploma, and healthcare experience. Travel and a valid driver's license are expected. Proficiency with mobile devices is required.

Qualifications

• Must have a high school diploma or equivalent.
• Minimum two years in health care with one year in home health or pediatrics.
• Knowledge of Medicare guidelines that govern home health or hospice.
• Basic technology skills including operation of a mobile device.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with branch and sales teams to ensure coordination and transition to services.
