Enhabit Home Health & Hospice seeks a Referral Coordinator to support patient referrals and act as a resource for referral sources. You will collaborate with the branch and sales teams to ensure smooth coordination and transition to services, while applying Medicare guidelines and regulatory processes.

The role requires a high school diploma, 2+ years in health care with 1 year in home health or pediatrics, strong customer service, and basic technology skills including mobile devices.

Qualificações

• High school diploma or equivalent.
• At least two years in health care and one year in home health or pediatrics.
• Customer service experience required.
• Knowledge of Medicare guidelines for home health or hospice.
• Basic technology skills including use of a mobile device.
