Enhabit Home Health & Hospice is seeking a coordinator to manage patient referrals and support the transition to services. You will work with branch and sales teams, ensuring smooth coordination and adherence to guidelines.

The role requires two years in health care with at least one year in home health or pediatrics, strong customer service experience, and knowledge of Medicare guidelines. Basic tech skills including a mobile device are needed.

• High school diploma or equivalent.
• At least two years in health care and one year in home health or pediatrics.
• Experience in customer service.
• Knowledge of Medicare guidelines for home health or hospice.
• Basic technology skills, including operation of a mobile device.

Responsabilidades

• Coordinate patient referrals and act as a resource for referral sources.
• Collaborate with the branch and sales teams to ensure coordination and transition to services.
• Apply and demonstrate knowledge of regulatory guidelines governing processes.
