Job summary

Bluelake Homecare in Michigan is seeking an Intake Coordinator to guide clients in our pipeline through the approval process for care. You will gather documents, assist with Medicaid paperwork, and travel to client homes when families need help completing forms.

You will also participate in the in-home MDHHS assessment to determine eligibility and weekly hours, and prepare families so they can clearly describe their daily needs.

• Valid Michigan driver's license, reliable vehicle, current insurance.
• Able to pass a background check.

Bluelake Homecare is hiring an Intake Coordinator to work with clients already in our pipeline and get them approved for care.

Once a client enters intake, you will assist in: gathering documents, completing Medicaid paperwork, and traveling to client homes when a family needs help getting forms done right. You will also be present at the in-home MDHHS assessment that determines whether a client qualifies and how many hours of care they receive each week.

Nice to Have
