You will serve as the primary contact for clients and families, coordinate with Medicaid representatives, and maintain accurate records in the intake system while collaborating with care teams for smooth onboarding.

Qualifications

• Experience in homecare, home health, or healthcare intake coordination preferred.
• Knowledge of Michigan Home Help Program or Medicaid home care programs is a strong plus.
• Strong organizational skills with ability to manage multiple cases.
• Excellent verbal and written communication skills.
• Proficiency in EHR or intake management software.
