Job summary

Bluelake Homecare in Michigan is seeking an Intake Coordinator to guide clients in our pipeline through the approval process for care. You will gather documents, assist with Medicaid paperwork, and travel to client homes when families need help completing forms.

You will also participate in the in-home MDHHS assessment to determine eligibility and weekly hours, and prepare families so they can clearly describe their daily needs.

Qualifications

• Detail oriented and organized.
• Strong communicator, patient with seniors and families.
• Comfortable working independently in the field.
• Valid Michigan driver's license, reliable vehicle, current insurance.
• Able to pass a background check.

Responsibilities

• Move clients through intake to start of care.
• Collect and verify consents, physician paperwork, and Medicaid forms.
• Travel to homes to help families complete documents in person.
• Attend MDHHS in-home assessments and support the client and family.
• Prep families beforehand so they can clearly describe their daily needs.
