Home Choice MI, LLC is seeking an Intake Specialist to manage the client intake process for Michigan's Home Help Program. The ideal candidate understands enrollment requirements and can guide clients and families through the application with compassion and efficiency.

You will process referrals, verify eligibility, liaise with MDHHS caseworkers, and maintain accurate records while supporting a caseload in Oakland or Macomb County.

Qualifications

• Prior experience with Michigan's Home Help Program intake process (required).
• Familiarity with consumer-directed home care / Medicaid waiver programs.
• Located in or near Oakland or Macomb County.

Responsibilities

• Process new client referrals and intake applications for the Home Help Program
• Guide clients through eligibility verification and required documentation
• Liaise with MDHHS caseworkers and coordinate paperwork submission
• Maintain accurate, compliant client records
• Communicate program requirements clearly to clients and families
• Track intake pipeline and follow up on pending applications
