Job summary

Developmental Pathways is seeking an Intake Case Manager to support access to Medicaid Waiver and State General Fund services. You will perform in-home assessments, engage families and providers, and coordinate intakes from a hybrid Aurora-based role with remote and field work.

Expect travel within Arapahoe, Douglas and Elbert counties and regular virtual meetings. The position offers a 4-day, 40-hour workweek with a busy but rewarding schedule.

Qualifications

• Bachelor’s Degree; or Five (5) years of experience in LTSS/Develop­mental Disabilities
• Some combination of education and relevant experience
• Valid Colorado motor vehicle license and insurance
• Meet driving requirements - no suspensions and no more than two tickets in past 3 years

Responsibilities

• Conduct in-home visits to complete assessments and connect individuals to services
• Engage with families and providers in the community to build trust
• Collaborate at Aurora office with team and other departments
• Work from home with Zoom meetings and intake coordination
• Travel between Arapahoe, Douglas, and Elbert counties weekly as needed
• Timely response to referrals and ongoing eligibility assessments and coordination
