Job summary

Imagine! in Boulder County is seeking a Case Specialist to provide enhanced case management to residents enrolled in HCBS Waivers who have IDD, brain injury or autism and ongoing, intensive needs. You will prevent crises by thorough planning and advocacy.

You will work with DHS, SSA and community resources to ensure funding and benefits are maintained, maintain documentation, and participate in IP meetings and service plan revisions. A hybrid work arrangement may be available.

Qualifications

• Bachelor’s Degree in Human Services preferred.
• One year of experience as a Case Manager required.
• Experience with public benefit systems preferred.

Responsibilities

• Provide enhanced case management for a small caseload of residents enrolled in HCBS waivers.
• Attend regular case management team meetings to stay updated on rules and policies.
• Maintain Knowledge of Medicaid/SSI/SSDI and related benefits.
• Complete timely, accurate case notes and service plan revisions.
• Collaborate with DHS, SSA, service providers and other agencies.
