Job summary

Developmental Pathways (DP) seeks a Program Manager - Active Case Management to lead the Case Management team in Colorado. You will guide staff, ensure quality case management for individuals and families, and uphold confidentiality and due process requirements in a hybrid Colorado office setting.

The role requires Bachelor’s degree or LTSS experience, plus supervision experience and Medicaid knowledge. Competitive salary and comprehensive benefits support work-life balance.

Qualifications

• Bachelor’s Degree or 5 years in LTSS field ( Developmental Disabilities )
• Minimum 2 years of mentorship or supervisory experience
• Working knowledge of Medicaid Long Term Care system
• Valid Colorado driver’s license and insurance
• Ability to handle sensitive information with discretion

Responsibilities

• Supervise and direct activities of assigned staff to ensure timely processing and reporting
• Foster an inclusive team and maintain productive cohesion
• Evaluate staff performance and complete performance reviews
• Coach staff to improve documentation, deadlines and soft skills
• Lead project management efforts with emphasis on stakeholder communication and resource planning
• Ensure compliance with policies, procedures and regulatory guidelines for funded services
• Manage workloads and coverage during absences or vacancies
• Participate in hiring processes when needed
