Job summary

Developmental Pathways is seeking an Intake Case Manager to join our Systems Navigation team in Aurora, CO. This role involves in-home assessments and coordinating supports for individuals with disabilities.

You will travel within Arapahoe, Douglas and Elbert counties, participate in virtual meetings, and collaborate with multiple departments to ensure seamless access to services. A 4-day, 40-hour schedule with flexible remote options is offered.

Qualifications

• Bachelor’s Degree; or five (5) years of LTSS experience such as Developmental Disabilities.
• Valid Colorado motor vehicle license and proof of insurance.
• Meet driving requirements with limited suspensions/tickets.

Responsibilities

• Conduct in-home assessments and connect individuals to programs and services.
• Engage families and providers to build trust and partnerships.
• Collaborate at the Aurora office and with other teams to improve service access.
• Work from home and travel weekly within the catchment area to fulfill duties.
