Developmental Pathways (DP) is hiring a Case Manager - Active and Coverage in Aurora, CO. The role supports eligibility verification, service coordination, and crisis response for individuals with disabilities.

The position blends in-office work with remote and field duties across Arapahoe, Douglas, and Elbert counties. The ideal candidate holds a Bachelor’s degree (or 5+ years LTSS experience) and a valid CO driver license.

Qualifications

• Bachelor’s Degree; or five (5) years of experience in LTSS.
• Valid Colorado motor vehicle license and provide proof of motor vehicle insurance.
• Meet our driving requirements - no suspensions and no more than two (2) tickets in the past three (3) years)

Responsibilities

• Conduct in-home assessments, monitor service provision, and facilitate service plan meetings.
• Manage caseloads in coverage to ensure support is provided.
• Engage with families and providers to build trust and partnerships.
• Collaborate at the Aurora office and share insights with teams.
• Work from home with virtual meetings and comprehensive documentation.
• Travel weekly within catchment area to fulfill duties and responsibilities.
