Developmental Pathways (DP) seeks Active Case Managers to join our Case Management Team in Aurora, CO. You’ll deliver compassionate, person-centered service and coordinate Medicaid Waiver and State General Fund supports.

Your days will include in-home assessments, monitoring services, and facilitating service plan meetings. This hybrid role blends field visits with remote coordination, monthly Zoom meetings, and cross-department collaboration.

Qualifications

• Bachelor’s Degree; or five years of LTSS/Developmental Disabilities experience; or a combination.
• Valid Colorado driver’s license and proof of insurance.
• Knowledge of Medicaid waiver and state-funded programs is a plus.

Responsibilities

• Conduct in-home assessments and monitor service provision.
• Facilitate service plan meetings and coordinate with families and providers.
• Collaborate with Aurora office and other departments; travel weekly in catchment area.
• Host virtual meetings, document services, and participate in team collaborations.

Developmental Pathways (DP) is seeking Active Case Managers to join our Case Management Team!As an Active Case Manager, you’ll play a vital role in delivering compassionate, person-centered service and support coordination to individuals receiving Medicaid Waiver and State General Fund services. Your days will be filled with:Conducting in-home visits to complete assessments, monitor service provision, and facilitate service plan meetings.Engaging with families and providers in the community to build trust and foster strong partnerships.Collaborating at our Aurora office sharing insights and strategies with your team and other departments to enhance service delivery. Average once per month.Working from home, which includes hosting virtual Zoom meetings with individuals and their teams, documenting and coordinating services, and participating in virtual office hours, team meetings and collaboration, and more.Driving between locations, spending time in the community and at your home or chosen remote workspace. This dynamic, hybrid role offers a fast-paced and rewarding experience where you help weave together services that empower the lives of individuals with disabilities.ScheduleEnjoy a 4-day, 40-hour workweek (Monday–Thursday or Tuesday–Friday) with the flexibility to work remotely, in-office, and in the field, to include individual's homes.This position requires frequent in-person presence, typically in individual's homes, to support essential responsibilities such as monitorings, assessments, and other duties in alignment with state regulations and company policies. Candidates should be prepared to travel within our catchment area (Arapahoe, Douglas and Elbert counties) on a weekly basis to fulfill these requirements.
