Developmental Pathways (DP) is hiring a Case Manager - Active and Coverage in Aurora, CO. The role supports eligibility verification, service coordination, and crisis response for individuals with disabilities.

The position blends in-office work with remote and field duties across Arapahoe, Douglas, and Elbert counties. The ideal candidate holds a Bachelor’s degree (or 5+ years LTSS experience) and a valid CO driver license.

Qualifications

• Valid Colorado motor vehicle license and provide proof of motor vehicle insurance.

Job Location: Developmental Pathways Aurora Office - Aurora, CO 80012, Position Type: FT - 40 hrs/week, Education Level: Bachelors, Salary Range: $25.10 - $28.68 Hourly, Job Shift: Day, Developmental Pathways (DP) is seeking a Case Manager (CM) - Active and Coverage to join our Program Operations Team!

As a Case Manager - Active and Coverage, you’ll verify eligibility, monitor, and ensure services and supports are coordinated, as well as provide support in emergency and crisis situations. Your days will be filled with:

• Conducting in-home visits to complete assessments, monitor service provision, and facilitate service plan meetings.
• Managing caseloads in coverage to ensure support is provided.
• Engaging with families and providers in the community to build trust and foster strong partnerships.
• Collaborating at our Aurora office, sharing insights and strategies with your team and other departments to enhance service delivery.
• Working from home, which includes hosting virtual Zoom meetings with individuals and their teams, documenting and coordinating services, and participating in virtual office hours, team meetings, and more.
• Driving between locations, spending time in the community and at your home or chosen remote workspace. This dynamic, hybrid role offers a fast-paced and rewarding experience where you help weave together services that empower the lives of individuals with disabilities.

Schedule

Enjoy a 4-day, 40-hour workweek (Monday–Thursday or Tuesday–Friday) with the flexibility to work remotely, in-office, and in the field.

This position requires frequent in-person presence to support essential responsibilities such as monitoring, assessments, and other duties in alignment with state regulations and company policies. Candidates should be prepared to travel within our catchment area (Arapahoe, Douglas, and Elbert counties) on a weekly basis to fulfill these requirements. Flexibility and reliability in meeting these obligations are critical to success in this role.

Requirements

• Bachelor’s Degree; or
• Five (5) years of experience in the field of Long-Term Services and Supports; or (LTSS), which includes Developmental Disabilities;
• Some combination of education and relevant experience appropriate to the requirements of the position
• Valid Colorado motor vehicle license and provide proof of motor vehicle insurance
• Meet our driving requirements - no suspensions and no more than two (2) tickets in the past three (3) years

Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.

Preferred

• One (1) year of experience in developmental disabilities/long-term services and supports
• Able to speak, write, and interpret fluently in English and another language based on the business needs
• Ability to complete all typical case management responsibilities in fluent language/s
• Support with written translation of internal documents for use by stakeholders, as appropriate

Additional Responsibilities

• Manage caseloads in coverage to ensure support is provided, aligning with rules and regulations while using appropriate delegation
• Complete any coverage tasks for individuals in coverage, including required documentation related to monitoring and PMPM activities and other duties as assigned
• Ensure effectiveness of services delivered (as authorized by the service plan) and ensure identified outcomes are met; advocate for individual service needs
• Provide support with coverage email management, research, tracking, assignments, etc.
• Provide paperwork support as needed, such as revisions, CAFs, DocuSign, etc.
• Work with the Program Manager to determine missed monitoring among coverage
• Assist in completing hand-offs as it relates to coverage
• Serve as subject matter expert for applicable programs and services; serve as resource/point of contact for complex cases
• Provide care coordination services and interventions by referring, educating, negotiating, and mediating with the individual and external providers/agencies
• Complete accurate case documentation in appropriate tracking and databases; prioritize workload and meet all required timelines
• Complete required documentation related to monitoring activities and other targeted case management activities as PMPM; evaluate need and effectiveness of services requested and delivered
• Ensure accuracy and timely entry and distribution of required documentation to all necessary team members, including, but not limited to, providers, individuals, and family members
• Maintain knowledge of state/ federal regulations, policies, and procedures regarding long-term services and supports, Medicaid waiver and state-funded programs and other community resources
• Maintain a professional and ethical manner in all interactions and meet performance quality, customer service, and coordination standards as assigned by department leadership; maintain appropriate boundaries
