Job summary

Jefferson County Human Services seeks Long Term Care Case Managers to provide individualized client support, assess eligibility, and coordinate care plans. Roles span home visits, community assessments, and interagency collaboration to ensure optimal resource use and outcomes for clients with complex needs.

These positions require working hybridly across the Denver Metro area, with flexibility for remote and in‑office tasks.

Qualifications

• Bachelor’s degree in a related field and 1 year of case management experience.
• Ability to assess eligibility, develop care plans, and coordinate services.
• Bilingual Spanish is a valued asset and may be compensated.

Responsibilities

• Determine client eligibility by gathering information from hospitals and home health services.
• Conduct face‑to‑face assessments at home or community settings.
• Prepare legal documents for appeal hearings and participate as needed.
• Maintain and reassess client care plans for cost‑effective service delivery.
• Coordinate with Medicaid providers and other service providers.
