Job summary

Developmental Pathways is hiring a Non-HCBS Case Manager in the Systems Navigation department to ensure timely intake and coordinated access to non-waiver programs for individuals. The role requires coordinating eligibility assessments, connecting clients with providers, and supporting enrollment activities while adhering to state regulations.

Travel within Arapahoe, Douglas and Elbert counties is expected weekly.

Qualifications

• Bachelor’s Degree or higher in a related field.
• Five (5) years of experience in Long-Term Services and Supports (LTSS).
• Experience with developmental disabilities and LTSS programs is preferred.
• Valid Colorado driver license; proof of motor vehicle insurance; meet driving requirements.

Responsibilities

• Serve as intake and eligibility liaison for non-HCBS services.
• Coordinate eligibility assessments and connect individuals to services.
• Provide care coordination and resource navigation for individuals and families.
• Document case progress in appropriate systems; meet timelines and requirements.
• Maintain knowledge of state/federal LTSS regulations and program guidelines.

Job Location: Developmental Pathways Aurora Office - Aurora, CO 80012; Position Type: FT - 40 hrs/week; Education Level: Bachelors; Salary Range: $25.10 - $28.68 hourly; Job Shift: Day. Developmental Pathways (DP) has an opportunity for a Case Manager (CM) - Non-HCBS in our Systems Navigation (SN) Department. This role is responsible for ensuring a timely and responsive intake process for individuals seeking non-waiver program supports. The Non-HCBS Case Manager coordinates eligibility assessments, facilitates access to Non-HCBS programs, connects individuals with appropriate service providers, and supports Intake and Enrollment activities as needed to ensure a smooth and effective service experience.

Schedule

Enjoy a 4-day, 40-hour workweek (Monday – Thursday or Tuesday – Friday) with the flexibility to work remotely, in-office, and in the field.

This position requires frequent in-person presence to support essential responsibilities such as assessments, monitoring, and other duties in alignment with state regulations and company policies. Candidates should be prepared to travel within our catchment area (Arapahoe, Douglas, and Elbert counties) on a weekly basis to fulfill these requirements. Flexibility and reliability in meeting these obligations are critical to success in this role.

Requirements

• Bachelor’s Degree; or
• Five (5) years of experience in the field of Long-Term Services and Supports; or
• (LTSS), which includes Developmental Disabilities; or
• Some combination of education and relevant experience appropriate to the requirements of the position
• Valid Colorado motor vehicle license and provide proof of motor vehicle insurance
• Meet our driving requirements - no suspensions and no more than two (2) tickets in the past three (3) years
• Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.

Preferred

• One (1) year of experience in developmental disabilities/long-term services and supports

Additional Responsibilities

• Serve as subject matter expert for applicable programs and services; serve as resource/point of contact for complex cases
• Complete various assessments to determine eligibility for services/supports and identify individual strengths, needs, concerns, and preferences through interviewing, observing, and utilization of standardized tools
• Provide care coordination services and interventions by referring, educating, negotiating, and mediating with the individual and external providers/agencies
• Complete accurate case documentation in appropriate tracking and databases; prioritize workload and meet all required timelines
• Complete required documentation related to monitoring activities; evaluate need and effectiveness of services requested and delivered
• Schedule, facilitate, document, and complete follow-up for all required and ad-hoc meetings; disseminate updates to the necessary parties
• Ensure the accuracy and timely entry and distribution of required documentation to all necessary parties, including, but not limited to, providers, individuals, and family members
• Maintain knowledge of state/ federal regulations, policies and procedures regarding long-term services and supports, Medicaid waiver and state-funded programs, and other community resources
• Maintain a professional and ethical manner in all interactions and meet performance quality, customer service and coordination standards as assigned by department leadership
