Job summary

The Salvation Army USA Central Territory in Indiana seeks an Intake Specialist to assess clients for placement into treatment programs, complete required biopsychosocial and health risk forms, and develop initial treatment plans while coordinating with clinicians and external referral sources.

You will maintain confidentiality, participate in weekly MDT meetings, and use DSM V criteria to support diagnoses, with occasional evening/weekend shifts as needed.

Qualifications

• Master's degree or licensure track (LMHC/LCAC or LCSW) required.
• Two years of office experience preferred.
• Ability to maintain client confidentiality and work with diverse populations.

Responsibilities

• Assess clients for placement into treatment programs using standardized forms.
• Develop an Initial Treatment Plan and ensure releases are properly signed.
• Coordinate with primary clinicians and external referral sources.
• Enter client information into the CSM system and maintain records.
• Attend staff meetings and comply with training and certification requirements.
